Intake Paperwork
All adult clients or parents/guardians of minor clients are required to complete intake paperwork. This can be done via our patient portal or while in the office. To obtain access to the portal, please contact the office at (317) 252-5518 to request the portal invitation link.
Counseling Rates
$150.00 for Intake and 60 minute sessions.
$125 for 45 minutes
$100 for 30 minutes
Payment
We accept cash, check and all major credit cards as forms of payment. We also accept payment via the patient portal.
Insurance
Most therapists with CenterPoint Counseling are currently contracted (in-network) with Anthem Blue Cross Blue Shield commercial plans only and CareSource Marketplace plans. CenterPoint is considered “out-of-network” with all other insurance companies. Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your insurance to verify how your plan compensates you for psychotherapy services.
We would recommend asking these questions to your insurance provider to help determine your benefits:
- Does my health insurance plan include mental health benefits?
- Do I have a deductible? If so, what is it and have I met it yet?
- Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
- Do I need written approval from my primary care physician in order for services to be covered?
Medicare/Medicaid
CenterPoint Counseling does NOT work with or file claims with Medicare or Medicaid. Additionally, we do not file claims with any supplemental insurances associated with Medicare. We apologize for any inconvenience and encourage you to have a conversation with your therapist if you have financial concerns.
Samaritan Fund
In the event that costs are prohibitive to you getting the care you need, please have a conversation with your therapist. Funds are available to assist with fees should the need arise.
Cancellation Policy
If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you will be charged a $75.00 fee. This fee is not eligible for Samaritan Fund support and cannot be filed with your insurance.
Marriage Preparation Program
CenterPoint offers a Marriage Preparation Program that consists of three 1 hour sessions with one of our counselors. A workbook is also provided to complement the conversations you may have during and after your sessions.
This program is open to the community if desired, but is required for those getting married at Second Presbyterian Church or using a pastor from Second. The cost of this program is $400.00. (This program cannot be filed with your insurance)
