Patient Portal

Intake Paperwork
All adult clients or parents/guardians of minor clients are required to complete intake paperwork.  This can be done via our patient portal or while in the office. To obtain access to the portal, please contact the office at (317) 252-5518 to request the portal invitation link.

Access Your Portal Account
Utilizing the portal you time. The portal allows you to view upcoming appointments, fill out all needed/requested paperwork and view your balance. Please call the office with any portal questions. 

Make A Payment
Payment is due at time of service. While leaving a credit card on file is expected, payments can be made through the patient portal.  

Electronic statements are sent out every two weeks that also provide a convenient link to pay. please know that some balances may not always appear correctly if utilizing insurance or if other arrangements were made.
